Anonymous by design
This form does not ask for or store your name, email, account ID, IP address, questionnaire answers, or health context. Each report is scheduled for automatic deletion 90 days after it is received.
Do not include health or medical details, contact details, passwords, sign-in codes, saved links, or other account secrets. We cannot reply to an anonymous report, so keep the reference shown after submission.
Accessibility
This site aims to meet WCAG 2.2 Level AA. Every release is checked automatically for text contrast, accessible names on controls, heading and landmark structure, ARIA references, target sizes, and horizontal overflow, across desktop, tablet and phone widths at normal size, 200% and 400% page zoom, and 200% text-only zoom. Keyboard focus is walked with real Tab presses to confirm every control shows a visible focus outline.
Automated checks also cover the things a screen reader depends on that a machine can decide: whether every control's name says what it does, whether a visible label is contained in that name so voice control can address it, whether custom tabs and checkboxes declare their state, whether status messages sit in a live region so they are announced at all, and whether an error is tied to its field.
Two things have not been verified and we will not claim otherwise. The site has not been tested with a screen reader such as NVDA, JAWS or VoiceOver, and it has not been reviewed by a person who relies on assistive technology. No automated check can judge whether an announcement reads sensibly, or whether a flow is genuinely usable.
